CANCELLATION POLICY: You may cancel with a refund 30 days prior to the start of the workshop. There will be a $25.00 processing fee deducted from your deposit. Send an email to Marshalevey@bellsouth.net with CLASS CANCELLATION in the subject line. There will be NO REFUND after this date unless the class is filled by another student. Class space cannot be sold to someone who is not on the wait list and who has not been approved by the workshop coordinator.

RELEASE: As part of the consideration addition to the registration fee paid to the Florida Society of Goldsmiths for giving the course which I have registered. I hereby release the Florida Society of Goldsmiths Board of Directors, its members, instructors, as well as a person or entity on whose property such course may be given, from any and all liability for personal injury or property damage that suffer or sustain due to negligence, or otherwise, in connection with any such course, whether by ingress or egress, attendance or otherwise, Florida Society of Goldsmiths reserves the right to substitute instructors and course specifics if necessary. BY SUBMITTING THIS FORM YOU AGREE TO THE POLICY AND RELEASE.
